Requirements for Travel Documents
Applicants are advised that the Consular Officer reserves the right to require additional documents to establish the applicant’s identity, citizenship, and accurate personal data.
Please note that the Embassy does not provide photocopying or printing services. Applicants must bring all required originals and photocopies.

Travel Document Request Form
Travel Document
- Duly accomplished Travel Document Application Form and notarized at the Philippine Embassy.
Proof of Philippine Citizenship
- Original and four (4) photocopies of proof of current Philippine citizenship and identity, such as:
- Philippine passport* (if available)
- Permanent Resident Card or Pakistan Origin Card
- Dual Citizenship Identification Certificate
- Other government-issued identification documents
Proof of Urgent Travel
- Documentary evidence demonstrating the urgency of travel, such as:
- Medical certificate or hospital records
- Death certificate of an immediate family member
- Police report (in case of lost passport)
- Other relevant documents supporting the emergency request
Photographs
- Four (4) recent color photographs (2” x 2”), taken within the last six (6) months
- White background
- Front-facing, neutral expression
