Requirements for Report of Birth

A Report of Birth (ROB) must be filed for a child born abroad to at least one Filipino parent in order to register the birth with the Philippine Statistics Authority (PSA). All civil registry documents approved by the Embassy are transmitted to the PSA for registration in the Philippine civil registry system.

General Instructions
  • Present original documents and submit four (4) photocopies of each.
  • Kindly collate and arrange documents into four (4) identical sets, following the order below.
  • All entries in the Report of Birth form must be typewritten or printed legibly.
  • Entries must reflect the facts at the time of birth of the child.
  • Upon evaluation, the Consular Officer may require additional documents to determine the child’s citizenship, identity, or eligibility for registration under Philippine law.
List of Requirements
  1. Report of Birth Form (FA Form No. 40 / Rev. 01-24 April 2018)
  2. Birth Certificate Issued by the Country of Birth
  3. First Passport or Travel Document of the Child (if available)
  4. Parents’ Marriage Certificate
  5. Passports of Both Parents
  6. If Filipino Parent(s) Acquired Foreign Citizenship
  7. ROB Fee: PKR 18,000
Report of Birth Form (FA Form No. 40 / Rev. 01-24 April 2018)
  • Four (4) original forms printed on A4 paper
  • Must be originally filled out and signed with wet signature
  • Item 20 must be notarized by the Embassy Consular Officer
  • Entries must be typewritten or clearly printed
  • Original copy
  • Four (4) photocopies
  • If not in English, submit an official English translation

If required by the regulations of the country of birth, the birth certificate must be authenticated or attested by the Ministry of Foreign Affairs or the competent authority of that country.

  • Photocopies of the bio-data page (4 copies)
  • If unavailable:
    • Execute an Affidavit of Non-Submission of Document
    • Submit copy of any valid ID or recent passport, if applicable
  • PSA-issued Marriage Certificate with QR Code, if married in the Philippines; or
  • Report of Marriage (ROM) issued by a Philippine Foreign Service Post.
If parents are unmarried:
  • If the child will use the father’s surname:
    • Affidavit of Acknowledgment of Paternity (AAP) – executed by the father
    • Affidavit to Use the Last Name of the Father (AULF) – executed by the mother
    • These affidavits must be properly notarized and registered:
      • If executed in the Philippines – registered as Legal Instruments with the Local Civil Registry Office and authenticated by DFA
      • If executed abroad – registered with the Philippine Foreign Service Post exercising jurisdiction
  • If the child will use the mother’s last name:
    • Submit the mother’s Birth Certificate
Submit the following:
  • Passport valid at the time of birth of the ch ild
  • Current/valid passport of both parents
  • Four (4) photocopies of bio-data pages
If unavailable:
  • Execute an Affidavit of Non-Submission of Document
  • Attach copy of any valid ID, visa (at time of birth), residence permit, or work permit, if applicable
If parent(s) were naturalized abroad and subsequently re-acquired Philippine citizenship: Submit:
  • Certificate of Naturalization
  • Identification Certificate or Oath of Allegiance
  • Certificate of Re-acquisition/Retention of Philippine Citizenship
  • If the child was already 18 years old at the time of parent’s re-acquisition, submit Identification Certificate issued by the Bureau of Immigration
(For children more than one [1] year old at the time of filing)
In addition to the above documents, submit:
  1. Notarized Affidavit of Delayed Registration
  2. Notarized Affidavit of Two (2) Disinterested Persons

Approved ROBs are transmitted to the Philippine Statistics Authority for registration. Processing and availability of PSA copies in the Philippines are subject to PSA timelines. The Embassy does not issue PSA-certified copies.
